Helping clinical practices with the right integration tools

The Center of Excellence for Integrated Care (COE) assists practices with determining their readiness for making clinical, operational and financial changes to their practice, and then combines that level of readiness with a deep understanding of a practice’s current degree of integration.

We help with all things integrated care! We facilitate the development of buy-in among administration, clinicians, and staff, as well as hone the necessary team-based skill sets to succeed in integrated care. Clinics and healthcare systems are unique in their culture and their developmental needs with respect to integrated care and we work with you to develop the right services for your clinic.

COE provides assessment services to determine the degree of integration for primary care behavioral health integration and assessment services to determine the dual diagnosis capability for mental health and substance use integration.

We offer in-depth technical assistance services to provide hands-on strategic implementation support to advance integration efforts in individual practices, organizations and health systems.

Ongoing training is a critical component of successfully implementing whole person care services. It is important to support primary care and behavioral health providers with growing and developing competencies for providing whole person care, as well as supporting all team members in a clinic system with using whole person language and functioning as an integrated system.
