The Center of Excellence for Integrated Care (COE) helps practices evaluate and determine their readiness for implementing clinical, operational, and financial changes. We then align this readiness with an understanding of the practice’s current level of integration to facilitate those changes.

We help with all things integrated care, encouraging collaboration among administration, clinicians, and staff while improving the critical team-based skills needed for success.
