Center of Excellence for Integrated Care is a leading resource for integrated care practice assessment, training, and technical assistance.

For over a decade, the Center of Excellence for Integrated Care—a program of the Foundation for Health Leadership & Innovation—has been supporting health clinics and systems across North Carolina in providing whole-person care through workforce development, practice transformation, and advocacy.

Our approach is simple: we build buy-in across the organization and adapt evidence-based methods to fit local needs. We understand not all clinics have the same resources and tailor the core elements of evidence-based models and interventions to fit the needs of each clinic.

Our goal is to help clinics and providers improve the mental and physical health of primary care patients and clients of mental health home clients. We partner with various sites and programs, including primary care offices, hospitals, community collaboratives, school-based health centers, substance use facilities, mental health clinics, and local government organizations.

The mission of the Center of Excellence for Integrated Care is to enhance whole person health care across North Carolina through workforce development, practice transformation, and advocacy.
